SkyLens Imagery photographs youth sports. SkyLens Imagery is based in Temple, Texas. Parents and coaches register an athlete or team on our registration form at skylensimagery.studio/register, which they reach by scanning the QR codes on our signs at the fields (or the Season Pass form linked from it). Texts are optional: the form asks for a mobile phone number, and below it there is a separate, unchecked text box. Parents who leave it unticked can still register; they get their receipt and photo link by email only.

Below is that part of the form, live, with the exact wording parents see. The full form also asks for the athlete’s name, team and jersey number. A screenshot of the whole form is at skylensimagery.studio/sms-opt-in.png.
